Requirements

  • A bachelor's degree in a social-service-related field, or an associate degree and more than seven years of experience in the social service field.
  • Knowledge and experience in the human services delivery system and networking.
  • Computer proficiency in Microsoft Word, Excel, PowerPoint, and the Internet, with the ability to back up computer data daily.
Responsibilities
  • Manage intakes, case files, assessments, statistical reporting, service plans, and updates on residents’ progress.
  • Meet regularly with the Director and/or Resident Manager regarding resident intakes and progress.
  • Coordinate services with community resources for residents.
  • Maintain all Department of Human Services contracted requirements under the Emergency Shelter & Bridge Housing Scope of Service Guidelines.
  • Provide required data to the Department of Human Services or The Salvation Army Divisional Headquarters.
  • Maintain confidentiality of residents’ information.
  • Receive and process requests for admission to the Family Caring Center.
  • Establish and maintain contacts with human and community service agencies and institutions, including federal, state, and private organizations, to collaborate on services.
  • Maintain a statistical data-collection and processing system for Family Caring Center residents and referrals, including shelter inquiries.
  • Attend trainings and monthly and quarterly meetings.
  • Coordinate needed services for children.
  • Assess housing barriers for individuals and families experiencing homelessness to determine housing and service needs.
  • Work to reduce households’ average length of stay in emergency shelter.
  • Work to increase the number of households who exit to permanent housing.
  • Assist participants in developing a strength-based, solution-focused individualized housing plan that promotes obtaining and maintaining permanent housing.
  • Develop an effective and timely referral network to ensure ongoing direction and support as needed.
  • Meet with residents weekly to update service plans and short- and long-term goals.
  • Identify participants’ strengths and barriers to stability, and assist them in reducing barriers and connecting with resources and services.
  • Provide mediation and advocacy with landlords and housing providers on clients’ behalf to develop workable plans to obtain or maintain housing.
  • Assist participants in locating and securing housing.
  • Create and maintain consistent verbal and written communication channels among tenants, landlords, referral sources, and collaborating agencies.
  • Provide information and referral assistance regarding support available from appropriate social service agencies and community programs.
  • Use the Resource Library to provide budget counseling and education, assist clients in establishing payment plans for bills and past debts, and help clients obtain and maintain housing.
  • Apply knowledge of residential lease contracts to educate clients about their rights and responsibilities.
  • Maintain accurate case notes, monthly outcome reports, and client files according to contract requirements, including client entry and exit in HMIS.
  • Collect and report program data, including HMIS reporting and funder-required data.
  • Perform other duties as assigned.
