Requirements
  • Graduate of an approved school of professional nursing.
  • Current licensed Registered Nurse in the practicing state.
  • At least one year of experience and responsibility in clinical home health services, or equivalent experience.
  • Must hold a valid driver's license in the appropriate state and have independent transportation with current insurance coverage.
  • Demonstrated competence in applying processes that ensure adherence to pertinent federal and state rules and regulations.
  • Strong communication skills with staff, physicians, referral sources, patients, and their families.
  • Computer literacy skills.
Responsibilities
  • Administer the patient/client Care Plan and oversee the patient/client’s health and wellness as indicated by the Care Plan.
  • Ensure compliance with nursing and clinical standards, federal, state, and local regulations, and company policies.
  • Treat patients when appropriate and as needed for Skilled Nursing Services.
  • Follow the appropriate Care Plan in collaboration with the Director of Nursing, patient/client, caregivers, physician, and other care-team members.
  • Perform on-site orientation, quarterly supervision visits, skill checks, and annual performance evaluations of home-care aides in compliance with applicable regulations and company policies.
  • Provide ongoing instructions to home-health aides during field visits, including reinforcement of Standard Precautions, client respect, HIPAA, and safety.
  • Review Personal Care Plans from Aging Service Access Points and orient aides in following the care plan authorized by the ASAP RN.
  • Document services rendered and changes in client conditions or family and home situations.
  • Communicate concerns or issues regarding client status to ASAP Case Managers or the client’s legal representative or family as necessary.
  • Perform other duties as assigned.

  • Current CPR certification.

Associated Home Care is a provider of nonmedical and supportive home-care services. It helps older adults and other clients remain at home through personal care, homemaking, companionship, transportation, and related assistance. Families, caregivers, referral partners, and community programs use its local care teams for scheduled and ongoing support. Caregivers are coordinated through assessment, matching, scheduling, supervision, training, and branch operations.
