Requirements
  • Registered nurse with current licensure
  • Graduate of NLN accredited school of nursing with a BSN degree preferred
  • Previous experience in hospice/home care setting
  • Two (2) years management or supervisory experience
  • Excellent observation, good nursing judgment, and communication skills
  • Understanding of hospice philosophy and issues of death/dying
  • Understanding of principles of pain/symptom management
  • Licensed driver with insured automobile in good working order
  • Possess and maintain current BLS Hands on CPR Certification
Responsibilities
  • Receives case referrals
  • Reviews patient information and determines hospice/home care needs
  • Assigns appropriate hospice personnel to case
  • Conferences with MEDICAL DIRECTOR regarding eligibility for services
  • Reviews and evaluates each case through various means
  • Assists clinicians in establishing therapeutic goals and developing care plans
  • Attends case conference meetings with hospice personnel
  • Conducts quarterly record reviews and communicates findings
  • Assists in screening and interviewing new employees
  • Assists in orientation of new employees
  • Assists in planning, implementing, and evaluating education programs
  • Complies with professional standards and principles
  • Assesses health needs of community and assists in program development
  • Participates in quality assessment performance improvement activities
  • Performs other delegated duties
