Responsibilities
  • Coordinate move-in into housing units.
  • Serve as a liaison between tenant and community-based case management team.
  • Ensure tenant well-being and unit habitability through a regular schedule of unit inspections and wellness checks.
  • Provide assistance for critical issues that impact housing stability.
  • Respond to housing and tenant-related emergencies during normal business and after hours.
  • Identify and secure community resources in coordination with assigned case manager.
  • Gain expert knowledge of homeless populations, common barriers to housing, and overall knowledge of community resources.
  • Understand the building lease term agreement and provide education to tenants and case managers on lease requirements.
  • Maintain client related data tracking systems, including case notes and complete HMIS and Salesforce entries.
  • Prepare case-related reports, including, but not limited to, outcomes, successes, and challenges.
Desired Qualifications
  • Two (2) years of experience in a human services or behavior-related field.
