Requirements
  • Must possess a valid LPN license issued by the State School of Nursing.
  • Must possess, at a minimum, a high school diploma or a GED.
  • Must be able to read, write, speak and understand English.
  • Must provide annual verification of a negative TB skin test.
  • Proof of full COVID-19 vaccination is required, with exceptions for medical and religious accommodations.
  • Must be able to move intermittently throughout the workday.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to see and hear, or use prosthetics that enable these senses to function adequately.
  • Must meet the facility's general health requirements, including a medical and physical examination.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and sometimes agitated residents.
  • Must be able to push, pull, move, and/or lift at least 25 pounds to a minimum height of 5 feet and carry that weight at least 50 feet.
  • May be necessary to assist in evacuating residents during emergency situations.
Responsibilities
  • Provide care, treatment, and medication administration to assigned residents, continually monitor and assess residents' conditions, and report according to plans of care, established medical practices, applicable laws and regulations, and company policies and procedures.
  • Assist with oversight of ancillary care staff when the Wellness Director or Assistant Wellness Director is absent.
  • Provide care to assigned residents based on plans of care.
  • Assist with and/or administer medications while adhering to the Five Rights of Medication Administration.
  • Reorder and receive medications.
  • Provide personal care and activities of daily living assistance.
  • Supervise care staff.
  • Maintain shift-to-shift reports.
  • Provide emergency assistance to residents with appropriate follow-up.
  • Obtain and transcribe physician orders.
  • Maintain case management documentation.
  • Comply with federal, state, and local health and regulatory standards and guidelines.
  • Adhere to facility policies and procedures, attendance policies, and employment handbook terms.
