  • At least three years of recent, related experience working in health and wellness promotion, an inpatient setting, or another appropriate clinical setting.

Responsibilities
  • Identify, facilitate, and secure access to healthcare, social service benefits, and community resources.
  • Assist members with navigating the steps toward efficient and effective care.
  • Coordinate appointments and transportation to physicians and non-physician providers for timely delivery of diagnostic and treatment services.
  • Monitor incoming calls, conduct outgoing calls, and respond to voicemail requests in a timely manner, including requests related to gaps in care, health risk assessments, upcoming education or health events, and provider follow-up.
  • Identify and assess members’ medical, behavioral, social, emotional, and financial needs.
  • Use social, economic, behavioral, and support systems and programs to connect at-risk members with appropriate community resources and address barriers and adherence.
  • Conduct health education.
  • Build relationships with members, their families, and caregivers, and support members in achieving their healthcare goals.
  • Provide emotional support or refer members to community-based or physician/provider resources for a higher level of psychosocial intervention.
  • Educate assigned members and engage them in programs; complete interventions, identify members requiring complex interventions, and refer them to other programs or resources using established criteria and documentation processes.
  • Complete surveys and assessments for assigned members to support health and wellness needs and engagement in care programs.
  • Deliver condition-specific education, medication-adherence education, preventive-care guidance, and navigation of available health benefits.
  • Address gaps in care, reinforce provider care plans, and promote adherence to evidence-based practices.
  • Collaborate with interdisciplinary teams to support whole-person care, improve quality outcomes, and enhance the member experience.
  • Identify and report quality-of-care issues according to departmental policies and procedures.
  • Maintain member confidentiality.
  • Document care-navigation activities in the care-management documentation system according to established policies and procedures.
  • Attend company and departmental meetings and required training sessions.
  • Assist with assigned population processes, including retrieving and assigning referrals and completing monthly reconciliation reports between documentation systems.

The headquarters sits on a field he once rode past on his bike as a kid growing up in Susquehanna Township. That local connection isn't just sentimental - it's a daily reminder of the commitment he feels to the community that shaped him. As Capital's President and CEO, Shamash leads a company that serves more than 800,000 members across 21 counties in central Pennsylvania and the Lehigh Valley. Capital Blue Cross is more than just a health insurer - it's also a community partner working to improve access to effective, affordable care.
