Requirements
  • A valid Licensed Vocational Nurse license in the State of California is required.
  • Experience in long-term care community health, geriatric care, or rehabilitative aide work is required; experience with people with Alzheimer's disease or dementia is preferred.
  • Ability to read, write, speak, and understand English to assist in emergency situations.
  • Ability to understand, follow, support, and initiate facility policies and procedures, recognize emergency situations, and respond appropriately.
  • Successful criminal background clearance through the Department of Justice and Federal Bureau of Investigation.
  • A high school diploma or General Educational Development credential is required.
  • Current Cardiopulmonary Resuscitation, Basic Life Support, and First Aid certification is required.
  • Must be at least 18 years of age.
Responsibilities
  • Support the clinical and care needs of residents.
  • Administer, order, and refill medications; dispense medications and provide physician or ancillary treatments.
  • Provide care and assistance to residents living with Alzheimer's disease, dementia, or another memory impairment.
  • Act as a liaison and maintain positive relationships with residents, families, and physicians.
  • Assist older adults with mobility needs and activities of daily living, including bathing, grooming, and routine personal tasks.
  • Monitor residents for changes in condition.
  • Develop comprehensive care plans based on each resident's needs and complete required documentation.
  • Collaboratively support the Program Coordinator with supervision of care staff, volunteers, and other staff providing resident services.
  • Maintain current task lists for care staff and update them when activities of daily living change.
  • Provide first aid and emergency assistance within the community.
  • Monitor and record residents' blood glucose readings according to their service plans.
  • Complete incident reports and participate in developing corrective action plans.
  • Maintain an electronic log of vital information using PointClickCare.
  • Follow established safety and infection-prevention protocols.
  • Work scheduled shifts punctually and reliably.
  • Follow company and department policies and procedures in the Employee Handbook.
  • Support other community departments when needed.
  • Complete admissions, discharges, and transfers as necessary.
  • Perform other duties assigned by the Director of Nursing.
Desired Qualifications
  • Experience with electronic documentation systems such as PointClickCare or a similar system.
