Requirements
  • Active nurse practitioner license and certification
  • Expert in diagnosis and treatment of patients with chronic and urgent medical/psychological conditions
  • Ability to travel locally at least 75% of the time
  • Excellent written and verbal communication skills
  • Strong computer skills, including experience with Electronic Health Records (EHR)
  • Consistently model Quartet’s values
  • Maintain high levels of patient satisfaction
  • Willingness to assist with after-hours call coverage
  • Experience in home-based services, complex care cases, or caring for patients with co-occurring substance use disorders is preferred
  • Experience in value-based care models is preferred
  • Previous telehealth experience is preferred
  • Skilled at using online tools and technology to deliver care
  • Dual FNP / PMHNP background is preferred
  • Highly collaborative and effective working in a team setting
  • Bilingual candidate (Spanish and/or other languages) is preferred
  • Comfort providing evidence-based care while being accountable for clinical outcomes and quality
  • Passionate about helping build innovative models of care
Responsibilities
  • Serve as the 'Quarterback' of the Care Team
  • Provide expertise in general medicine and evidence-based behavioral health care
  • Perform Initial Intake visits with patients
  • Order, interpret, and analyze diagnostic tests
  • Prescribe and manage medications and treatments
  • Capture diagnosis details and submit claims
  • Identify social needs and non-medical barriers to care
  • Manage psychosocial component of patient care
  • Engage with interdisciplinary team for efficient care planning
  • Initiate internal referrals within the Quartet Care Team
  • Develop rapport with diverse set of patients
  • Uphold clinical customer service through high-quality care
  • Document all clinical findings and services in the electronic health record
