Requirements
  • A valid state LPN license is required.
  • A valid driver’s license and use of an automobile are required because of the travel involved.
  • Annual MUI incident training is required as dictated by the State of Ohio Medicaid, Ohio Home Care Program, and Department of Developmental Disabilities.
  • The employee must be able to read and understand agency documents, including Plans of Care, flow charts, client charts, policies, and procedures.
  • The employee must be able to use a computer, laptop, or tablet.
  • The employee must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
  • The employee must provide client care within agency and program rules.
Responsibilities
  • Administer medication in accordance with standards of practice.
  • Encourage social and rehabilitative skills, self-care, and independence.
  • Perform client treatments in accordance with the Plan of Care.
  • Assist consumers in learning appropriate self-care techniques.
  • Prepare clinical and progress notes documenting care provided.
  • Monitor and document client conditions and report changes to the Clinical Director or Clinical Supervisor.
  • Promote activities that achieve a safe and healthy environment.
  • Use aseptic technique when performing client procedures and care.
  • Collaborate with the clinical team to develop and improve the Plan of Care.
  • Assist clients in achieving established goals in the RN-developed Plan of Care.
  • Maintain productivity according to agency standards.
  • Maintain the required license, continuing education units, and mandatory employment requirements.
  • Participate in client care meetings, in-services, orientation, and staff meetings.
  • Provide services in accordance with the agency’s philosophy, standards, policies, and scope of practice.
  • Participate in Performance Improvement and adhere to HIPAA guidelines.
  • Perform other duties as assigned.
