Requirements
  • A Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field approved by Medicaid or required by the applicable funding source.
  • A minimum of zero years of experience in case management or service coordination with a Bachelor's degree.
  • Working knowledge of the mental and physical characteristics of the assigned population group, including growth and development, human behavior, treatment and service options, and related issues.
  • Working knowledge of SCS programs and services, policies, and procedures.
  • Working knowledge of applicable regulations and licensure standards.
  • Good interpersonal skills and written and oral communication skills.
  • Ability to lift up to 20 pounds, with frequent lifting or carrying of items weighing up to 10 pounds, or ability to perform significant periods of walking or standing.
  • Reliable transportation for frequent travel.
  • Availability for possible 24-hour on-call responsibilities.
  • Ability to work within applicable safety-management policies and Joint Commission standards.
Responsibilities
  • Conduct and document comprehensive assessments of clients and their needs.
  • Participate in developing client service plans consistent with assessments.
  • Arrange delivery of services identified through assessments.
  • Assist clients in accessing needed services through referrals or linkages.
  • Monitor client progress through referrals, appointment tracking, follow-up on services rendered, and periodic reassessments of changing needs.
  • Perform advocacy activities on behalf of clients, including writing letters to expedite entitlement payments and encouraging clients or family members to advocate for themselves.
  • Prepare and maintain case records documenting contacts, needed services, reports, client progress, and related information in accordance with SCS and regulatory standards.
  • Provide case consultation with service providers and collateral contacts to determine client status and progress.
  • Communicate with supervisors about case-management issues, resourcing, service collaboration, and development of new resources.
  • Provide psychosocial education to clients and family members to increase their understanding of the client's illness, physical condition, or social situation.
  • Perform crisis assistance, including intervention on behalf of clients, arranging emergency referrals, and coordinating other needed emergency services.
  • May serve as Primary Case Coordinator, with assignment volume and complexity depending on skill and autonomy.
  • Exercise principle-centered leadership focused on customer-service responsiveness and continuous quality improvement.
