Requirements
  • The registered nurse must hold an active license in good standing from the State Board of Nursing in the state of employment.
  • American Health Association Healthcare Provider CPR certification is required, or must be obtained within thirty days of hire, with biannual recertification.
  • Formal training in management of the aggressive patient is required within sixty days of hire, with annual recertification.
  • At the Hospital’s discretion, the employee may be required to hold a qualified medical professional designation and must meet the associated competency requirements.
  • Employees in Child/Adolescent Day Treatment programs licensed under HFS §40 must have training in psychiatric nursing, including work with children with mental health disorders, or one year of clinical experience working with these children.
Responsibilities
  • Complete initial assessments and document them as required.
  • Collect, record, and analyze pertinent admission-assessment data within the prescribed timeframe according to hospital policy, including patient strengths and limitations, cultural, spiritual, and ethnic factors, discharge needs, medical and physical status, medication compliance and reactions, current medication schedules, and age-specific needs.
  • Involve patients’ support systems, including family and friends, in assessment and documentation when appropriate.
  • Observe and document patients’ interactions with family and friends when pertinent to treatment, and obtain relevant history and individual-needs information from support systems.
  • Explain patient rights, obtain required signatures and consents, protect patient privacy and confidentiality, and assist with patient orientation.
  • Follow hospital policies and procedures for unit safety, including gown and contraband searches on admission, while remaining sensitive to patient and family stressors.
  • Initiate and update treatment plans and documentation as required.
  • Participate in planning and modifying patients’ plans of care, evaluate multidisciplinary assessment data, participate in care conferences, represent nursing care at staffings, and develop and interpret plans of care with patients and families.
  • Write clear, concise, and attainable treatment goals and review treatment plans as goals are achieved, changed, or updated.
  • Identify, interpret, and document information from nursing interviews, observations, physical assessments, and diagnostic data, and confer with other healthcare professionals as appropriate.
  • Review current laboratory data and follow up with the physician.
  • Evaluate fall potential and initiate fall precautions when indicated.
  • Identify potential self-abuse, suicidality, assaultive behavior, abrupt changes, impending instability, and significant cardiac or respiratory symptoms requiring immediate intervention.
  • Develop age-appropriate interventions for patients’ plans of care and document interventions when patient status changes.
  • Administer medications safely and accurately using the correct patient, medication, dose, time, and route.
  • Maintain knowledge of each medication’s purpose and effects, accurately document administration and patient responses, transcribe and implement physician medication orders, and monitor for adverse reactions, drug interactions, and other unexpected consequences.
  • Provide and document patient education about medications and maintain current knowledge of new pharmacologic products and therapeutic uses.
  • Use appropriate de-escalation interventions, including a quiet room, locked seclusion, and restraints, and reevaluate patient safety levels.
  • Identify alcohol withdrawal syndrome and extrapyramidal side effects or neuroleptic malignant syndrome signs.
  • Identify patient and family health-education needs, implement age-appropriate teaching interventions, and document education in the patient record and treatment plan.
  • Organize and implement patient-care activities according to patient priorities, preferences, unit needs, time constraints, established care plans, hospital policies, unit standards, age-specific needs, and cultural needs.
  • When requested and properly designated, ensure admissions, transfers, and related patient-care activities comply with the Emergency Medical Treatment and Labor Act and associated regulations.
  • Adhere to nursing-department and hospital standards of nursing practice and patient care, and protect patients, visitors, and staff from environmental hazards through safety and infection-control practices.
  • Participate in continuing education, in-service programs, performance improvement, and reporting relevant education to unit staff, including attendance at all required in-services.
  • Assist in developing and implementing unit standards of care, including patient safety levels, unit safety and hazardous-item procedures, Kardex documentation, and standard care plans.
  • Identify problems with unit systems, communication patterns, and resources affecting patient care and suggest solutions to the Clinical Services Manager.
  • Identify unit educational equipment and supply needs, serve on a unit-based committee or participate in unit projects, and participate as a project leader.
  • Participate in nursing and hospital committees and designated meetings, and evaluate and upgrade unit standards of care.
  • Assume the charge nurse role by coordinating unit activities, leading during crises, facilitating admissions, communicating unit status and projected needs, managing the therapeutic milieu, and exercising sound clinical judgment and decision-making.
  • Plan patient-care assignments based on patient needs, unit activity, nursing staff qualifications, and continuity-of-care guidelines.
  • Ensure the unit remains orderly and serve as a role model for communication, teaching, and leadership skills.
  • Supervise and delegate tasks to LPNs and psychiatric technicians based on their preparation and demonstrated abilities.
  • Provide complete acuity monitors to the Patient Accounts department to support accurate billing.
  • Maintain a primary caseload and direct patient care according to unit standards.
  • Develop professional-growth goals with the Clinical Services Manager, pursue continuing education, discuss progress toward improvement goals, and seek projects or extra tasks based on unit needs.
Desired Qualifications
  • A Bachelor’s degree in nursing is preferred.
  • Previous psychiatric experience with children, adolescents, and adults is preferred.
