Requirements
  • A current, unencumbered license for the state of Ohio to practice as a Registered Nurse or Licensed Practical Nurse is required.
  • One year of experience working as a nurse in a hospital, long-term care facility, or other related health care facility is required.
  • Proficiency with Windows, Microsoft Office (Word, Excel, and PowerPoint), and the internet is preferred.
  • Proficiency with electronic medical records is preferred.
  • Must be able to read, write, speak, and understand the English language.
Responsibilities
  • Meet with each resident regularly to provide health and wellness checks and answer health-related questions.
  • Review resident information and complete resident assessments before move-in, at move-in, when the resident’s condition changes, and as required by residence policy and state regulations.
  • Review each resident’s medical report before move-in to ensure the residence can meet individual needs and regulatory requirements.
  • Order medications and oversee medication schedules for residents receiving medication administration services.
  • Ensure proper documentation for medication assistance and administration.
  • Develop and maintain an up-to-date individual service plan for each resident.
  • Maintain ongoing communication with residents, families, and physicians regarding residents’ medical needs.
  • Coordinate additional health services residents may need.
  • Assist with coordinating resident transfers to and from hospitals or other facilities.
  • Assist with counseling residents and families.
  • Be on call for medical questions and resident emergencies as scheduled.
  • Assist with coordinating residents’ dietary needs with the Nutrition Coordinator.
  • Review changes in residents’ conditions with the Executive Director and residents’ physicians.
  • Assess residents and monitor their health.
  • Ensure that all infection-control guidelines are met.
  • Maintain residents’ wellness files.
  • Facilitate Occupational Safety and Health Administration training and assist employees with Occupational Safety and Health Administration compliance.
  • Oversee and assist with training regarding residents’ health and wellness.
  • Coordinate state-required training with the Executive Director to ensure regulatory compliance.
  • Attend regular staff meetings and required training sessions.
  • Attend community events.
  • Participate in the Manager On Duty program.
  • Communicate resident information on a need-to-know basis to other team members according to residence policies and procedures.
  • Notify a supervisor or the Clinical Director when observing a change in a resident’s condition.
  • Ensure compliance, excellence, and delivery of resident care in adherence with organizational, local, state, and federal regulations.
  • Maintain a positive and professional manner toward residents, visitors, families, and coworkers.
  • Adhere to all Ohio Living policies and procedures.
