Requirements
  • Current unrestricted California Occupational Therapist license.
  • Current Basic Life Support and cardiopulmonary resuscitation certification.
  • Valid California driver's license.
  • Knowledge of local community resources.
  • Knowledge and understanding of individual development and human behavior as they relate to the effects of illness and injury, and of the influence of culture on health care.
  • Self-direction, empathy, critical thinking, interpersonal skills, leadership skills, and organization.
  • Completion of a computer literacy and competency assessment before scheduling an in-person interview, including electronic charting and use of the web-based communication application.
Responsibilities
  • Partner with the Case Manager to provide skilled occupational therapy services in patients' homes.
  • Evaluate and address individuals' needs so they can live safely and productively in their home environment.
  • Provide services involving home safety, medication management, wheelchair seating and positioning, pain management, falls prevention, chronic disease and dementia management, and family caregiver training.
  • Serve as an educator, patient advocate, and member of the interdisciplinary care team.
  • Use electronic charting and web-based communication tools.
  • Provide occasional coverage outside the primary territory when needed for vacancies, leaves of absence, higher patient census, or other client care needs.
