Requirements
- A Bachelor's Degree in pharmacy from an accredited college of pharmacy is required.
- An Arizona State Board of Pharmacy license that is active and in good standing is required.
- Basic Life Support certification is required.
- Advanced Cardiovascular Life Support certification is required.
- Pediatric Advanced Life Support certification is required for positions working in Emergency and Pediatrics departments.
- Candidates should have an understanding of computers and competence using computers and basic software programs.
Responsibilities
- Perform clinical services to optimize drug therapy and document clinical interventions.
- Provide medication therapy management using medication-related protocols, including pharmacokinetics, parenteral nutrition, serum creatinine, and other therapeutic drug monitoring, by utilizing clinical software and assessing relevant information.
- Advise physicians on the use of formulary medications and suggest alternative therapeutic options.
- Provide accurate, clinically relevant drug information to physicians, nurses, and allied health professionals.
- Participate in code arrest response by providing information and assuring the availability of medications.
- Identify, prevent, and investigate potential and real adverse drug reactions and medication errors.
- Provide information required for drug-related patient counseling.
- Review and properly interpret physician orders, screening for incompatibilities, interactions, duplicates, and contraindications.
- Contact nursing personnel and/or prescribers regarding questions on drug orders.
- Dispense and label medication orders, including oral medications and intravenous solutions.
- Use pharmacy technology products such as electronic health records, automated dispensing machines, compounders, unit dosing, tube systems, and clinical decision support and data-mining tools.
- Process patient charges and credits.
- Assist in maintaining basic drug supplies from vendors.
- Exercise applicable legal and professional controls on controlled substances.
- Verify physical inventory for incoming and outgoing items.
- Properly document waste from returned, expired, or unusable controlled substances.
- Supervise and provide oversight of licensed and non-licensed supportive personnel, organizing and directing their work as needed.
- Report adverse drug reactions, medication errors, and good catches.
- Assist in training new staff members, including pharmacists and pharmacy technicians.
- Adhere to regulatory and organizational standards affecting patient care and safety, as well as hospital and departmental policies and procedures.
- Stay current with and comply with state and federal regulations, statutes, and company policies affecting the employee's area of responsibility.
- Ensure all certifications and licenses are up to date and valid before their expiration dates.
- Complete mandatory company modules and required job-specific training within the specified time frame.
- Report safety-related incidents promptly through the Midas/RDE tool, attend safety-related training programs, perform work safely, and monitor the work environment for safety issues.
Desired Qualifications
- A Doctor of Pharmacy degree from an accredited college of pharmacy is preferred.
- Three months to one year of experience is preferred.
