Requirements
  • A Master's Degree in Nursing or a related field is required, although applicable experience may substitute for education.
  • Three years of professional nursing experience are required.
  • Applicants must be able to communicate effectively in English, both verbally and in writing.
  • Applicants must have effective interpersonal skills, leadership skills, and clinical expertise.
  • Applicants must be able to access information and document in an electronic medical record.
  • Current Washington State Registered Nurse licensure is required at the time of hire.
  • Current Basic Life Support certification is required.
Responsibilities
  • Identify and monitor patients with high medical needs to ensure timely completion of consults, procedures, and tests, maintain ongoing team communication, and facilitate coordination between providers across the continuum of care.
  • Lead medical team and nursing huddles and work to remove barriers to discharge caused by medical complications or system or team obstacles.
  • Assist with ordering complex medical equipment and ensure safe transitions of care.
  • Develop and monitor outcome measures related to care coordination and staff and patient satisfaction with discharge planning.
  • Provide ongoing consultation and education to staff to improve patient flow through the UW Medical Center system.
Desired Qualifications
  • Experience in case management is strongly preferred.
