  • Master's Degree in Social Work or equivalent behavioral health degree eligible for licensure through the CA Board of Behavioral Sciences or approved by program funder (e.g., MSW, MFT)
  • Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable populations
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Successfully complete the following as a condition of hire:
  • Tuberculosis Test
  • Background Screening
  • Drug Test
Responsibilities
  • Provide leadership to the multidisciplinary teams, ensuring continuity of care and services between PATH agency resources and partnering agencies
  • Provide clinical oversight of staff and ensure that all program participants receive the services as identified in each participant service plan
  • Create and work within a team environment to ensure staff is knowledgeable and familiar with contract expectations, case management and supervisorial expectations and ensure staff is delivering high quality care
  • Provide individual and team supervision, support, and performance evaluations for assigned staff
Desired Qualifications
  • Three (3) years of experience in healthcare, case management, homeless services or working with vulnerable populations
  • One (1) year of supervisory experience in homeless services or related social services field strongly preferred
