Successful candidates will have the following: 

• Degree in Human Services or Social Work is preferred
• Compassion for an underserved population
• PCC experience
• Social work experience in a LTC setting

Your day to day: 

You’ll provide assistance in caring for our residents by assist with care planning, social programs, and familial relations.You’ll work under supervision of our Social Services Director to ensure compliance with current state, federal, and local standards.
