Requirements
  • A high school diploma or GED is required.
  • Excellent oral and written communication skills and the capacity and willingness to learn to use computer systems are required.
  • A valid driver's license is required, with the ability to drive a large vehicle, including a wheelchair-accessible or 15-seat van.
  • The candidate must demonstrate a teamwork philosophy and professional conduct consistent with agency standards.
  • The candidate must show respect and consideration for the residents served.
Responsibilities
  • Provide and ensure a safe, clean, normalized, home-like environment and inform the Program Manager of physical plant needs.
  • Promptly inform the Program Manager or Assistant Program Manager of serious incidents or violations of individuals' or staff members' rights according to established policies.
  • Develop activities of daily living and independent living skills for the individuals served.
  • Maintain and submit timely, accurate records and reports concerning individuals' development status, progress, daily goal data, monthly progress notes, summaries, and other required information.
  • Implement treatment plans by training residents in activities of daily living, food planning and preparation, cooking, house and room maintenance, budgeting and money management, recreation, and leisure activities.
  • Administer medication to assigned individuals with appropriate training and supervision and in compliance with management, medical, and nursing policies and procedures.
  • Communicate pertinent individual and program issues to staff and supervisors verbally and in writing, including through incident reports and the residence log book, and read the residence log book daily.
  • Participate in interdisciplinary treatment meetings and scheduled individual reviews as required.
  • Maintain ongoing knowledge of each individual's medical, psychological, cognitive, and social needs and ensure programming is appropriate to their needs and treatment goals.
  • Report observed or known deficiencies, inability to provide habilitation services, abuse, facility problems, and physical plant or safety issues to management immediately.
  • Maintain a clean environment inside and around the exterior of the facility, including rooms, bathrooms, kitchen, laundry, and garbage removal.
  • Transport individuals to and from programs, community inclusion activities, medical appointments, home visits, and other activities as needed.
  • Complete the Staff Information Entry Form when escorting individuals to medical appointments.
  • Provide habilitation services to each individual as prescribed in their service plans, including CFA, Individual Service Plan, and Habilitation Plan.
  • Assist individuals with skill development in activities of daily living, domestic living, recreation, and socialization.
  • Ensure assigned individuals' nutritional needs are met according to prescribed diets and assist with feeding as necessary.
  • Organize, assist with, supervise, and when necessary prepare meals for individuals.
  • Assume responsibility for agency or individual funds personally assigned for individual or program purchases.
  • Escort individuals to medical appointments as needed.
  • Meet attendance standards according to assigned shifts and the direction of the Assistant Program Manager or Program Manager.
  • Successfully participate in and pass medication certification courses and demonstrate continued proficiency in medication administration.
  • Perform related duties assigned by the Assistant Program Manager or Program Manager.
  • Maintain awareness of and sensitivity to the cultural and socioeconomic characteristics of the persons and populations served.
Desired Qualifications
  • AMAP Certification, SCIP-R, CPR, and First Aid are preferred.
