Requirements
  • Bachelor’s degree in social work or a related field preferred
  • Two or more years of experience in social service delivery, ideally within senior or family populations
  • Strong working knowledge of community resources and supportive services
  • Demonstrated ability to advocate, organize, problem-solve, and deliver consistent results
Responsibilities
  • Coordinate programs, services, and activities for residents
  • Provide support across both communities while following the assigned weekly schedule
  • Assist residents with applications for support programs such as Medicaid, Medicare, and other benefit programs
  • Organize and lead social events, engagement activities, and educational opportunities
  • Engage residents daily to identify needs and provide appropriate referrals
  • Serve as a liaison to community agencies and service providers, building a strong network of partnerships
  • Identify affordable service providers and negotiate discounts when possible
  • Develop and maintain a Resource Directory of state and local agencies
  • Offer educational sessions on health, wellness, life skills, and available support services
  • Deliver excellent customer service by supporting residents with basic needs and concerns
  • Assist residents in accessing services such as transportation, meals, personal support, financial assistance, and more
  • Document and report resident or family concerns as required
  • Maintain positive and professional relationships internally and externally
