Requirements
  • Hold a current Certified Medical Assistant or Certified Clinical Medical Assistant certificate issued by a recognized credentialing body.
  • Have at least two years of healthcare experience.
  • Have at least one year of telephonic or call center customer service experience.
  • Have experience working in a medical practice or healthcare setting, including familiarity with electronic health record platforms for documenting patient interactions and scheduling appointments.
  • Have experience and training in motivational interviewing and patient education, and be comfortable discussing serious illness, chronic disease, and end-of-life-adjacent topics with patients and families.
  • Have strong organizational skills and the ability to work independently while managing a high-volume outreach workload.
  • Be proficient with Microsoft and Google applications and have access to a reliable internet connection.
  • Have strong written and verbal communication skills.
  • Have exceptional interpersonal skills, including conflict resolution and de-escalation techniques.
  • Be willing and able to work Monday through Friday from 8:30 a.m. to 5:00 p.m. Eastern Standard Time, with the possibility of overtime.
  • Reside in one of the approved states: California, Georgia, Texas, Arizona, Colorado, Florida, Illinois, Nevada, North Carolina, Oregon, Utah, or Washington.
  • Complete identity and work eligibility verification through E-Verify upon hire.
  • Be vaccinated against influenza for the patient-facing role, subject to consideration of religious or medical accommodations.
Responsibilities
  • Conduct targeted telephonic outreach to patients on provided lists using standard messaging while delivering customer service and patient-centered communication.
  • Confirm service-area eligibility and complete structured screening forms capturing demographic information, chronic disease history, and indicators of functional decline to determine program eligibility.
  • Educate patients and families about palliative care, the organization's services, and what to expect from the program by translating complex clinical concepts into accessible language.
  • Schedule eligible patients for an initial visit with a nurse practitioner and coordinate logistics for a smooth handoff from enrollment to care.
  • Act as a liaison between patients and the broader care team by directing calls, texts, and emails to appropriate individuals or departments and responding to patient questions and concerns.
  • Maintain confidentiality and accuracy in patient documentation and communications in accordance with HIPAA and organizational standards.
  • Coordinate enrollment efforts across targeted regions, track outreach activity systematically, encourage referrals, and support census growth.
  • Identify outreach trends, surface barriers to enrollment, and share qualitative and quantitative feedback and ideas with the market and business development team to improve program reach.
  • Maintain a collaborative working relationship with the designated market team and practice leadership while contributing to program expansion and patient access.
Desired Qualifications
  • Eight months of previous clinical experience is preferred.
  • Call center experience is highly preferred.
  • Bilingual English and Spanish proficiency is preferred.
  • Medical terminology certification is preferred.
  • Familiarity with palliative care, home-based care models, or chronic disease management is preferred.
