Requirements
  • Active registration in good standing with the College of Nurses of Ontario as a Registered Nurse.
  • Current CPR and First Aid certification from a recognized provider, completed within the past 12 months.
  • Valid WHMIS 2015 certification issued or renewed within the past 12 months.
  • A negative TB test completed within the past 12 months, using a two-step test or a one-step test with prior documentation.
  • Proof of full COVID-19 vaccination or an exemption under the Ontario Human Rights Code.
  • A clear Criminal Record Check with Vulnerable Sector Screening issued within the past 12 months.
  • Ability to communicate effectively in English, orally and in writing, for documentation and communication with clients and care teams.
  • Comfort and capability providing care to male and female clients of varying ages and care needs.
  • A valid driver's licence and reliable access to a vehicle for daily tasks.
  • Ability to identify and report client care concerns to Care Coordinators.
  • Demonstrated experience delivering in-home care in community settings.
  • Ability to work flexibly and adapt, with organizational skills.
  • Ability to take initiative and adapt to rapidly changing demands.
  • Interpersonal, oral and written communication, and critical-thinking skills.
  • Demonstrated ability to work independently and in a team environment.
  • Availability for 10-12-hour shifts and the scheduling commitment applicable to the selected full-time or part-time arrangement.
Responsibilities
  • Complete comprehensive client assessments, develop care plans, and implement and evaluate care based on evidence-based practices and each client's needs.
  • Act as the point-of-care lead and remain accountable for a client's care when designated as the primary provider for a caseload, collaborating with care team members and support functions.
  • Support clients through care transitions by collaborating with and connecting them to local community support services.
  • Administer oral, topical, subcutaneous, and G-tube medications within the College of Nurses of Ontario scope and documented medical orders, and document administrations in the electronic Medication Administration Record.
  • Monitor and assess vital signs, document findings, and report abnormal readings or concerns to the designated Care Coordinator or physician.
  • Provide care involving oxygen therapy, G-tube feeding, and suctioning according to training and client-specific protocols.
  • Assist clients safely with mobility and transfers using mechanical lifts or manual assistance, following mobility plans, body mechanics, and safety procedures.
  • Perform prescribed range-of-motion and physical therapy exercises under a physiotherapist's direction or care plan, and assist with ambulation indoors or outdoors when required.
  • Observe and document changes in clients' physical, cognitive, or emotional condition and escalate concerns as needed.
  • Maintain accurate and timely documentation, including progress notes, incident reports, and communication logs, in accordance with company and regulatory standards.
  • Use and dispose of personal protective equipment according to infection prevention and control protocols and Public Health Ontario guidelines.
  • Follow organizational policies for infection control, client safety, privacy, and professional conduct, and immediately report hazards or incidents to the supervisor.
  • Visit multiple clients throughout the Northumberland region during the workday.
Desired Qualifications
  • Previous experience supporting Ontario Health atHome clients.
  • Previous experience working as a visiting nurse in the community.
  • Gerontology competencies.
