- A Doctor of Dental Surgery or Doctor of Dental Medicine degree from a U.S.-accredited dental school or completion of an Advanced Education in General Dentistry or General Practice Residency program.
- An active Florida dental license is required.
- An active Federal Drug Enforcement Administration registration and applicable certifications are required.
- Two or more years of professional clinical experience are preferred.
