Requirements
  • Eight (8) years of experience in healthcare, case management, homeless services or working with vulnerable populations.
  • Six (6) years of supervisory experience in homeless services or related social services field strongly preferred
  • Six (6) years of experience in healthcare, case management, homeless services or working with vulnerable populations.
  • Four (4) year of supervisory experience in homeless services or related social services field.
  • Master’s Degree in a field of study eligible for licensure by the California Board of Behavioral Sciences (LCSW, LMFT, or LPCC).
  • Possess Clinical licensure with the California Board of Behavioral Sciences (LCSW, LMFT, LPCC) and be two (2) years post-licensure in order to be able to Clinically supervise.
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Successfully complete Tuberculosis Test, Background Screening, and Drug Test
Responsibilities
  • Leading, directing, and supervising associate directors, program managers, team supervisors, and direct service staff.
  • Integrating whole person care services in shelter, housing, and community settings.
  • Oversight of CalAIM programming, including Community Supports, Enhanced Care Management, Day Habilitation, Community Health Work.
  • Clinical oversight and audit of case management, housing and health needs assessments, care plan documentation and practices.
  • Development, growth, and integration of billed for services, including fiscal maximization strategies.
  • Experience with Populations of Focus, including people experiencing or at risk of homelessness, chronic health conditions, mental health and substance use, justice involved.
  • Healthcare grant development, management, and reporting.
  • Leading and growing multi-disciplinary teams.
  • Funder and stakeholder liaison at a senior level.
