Requirements
  • A Master of Science or Master of Arts degree in social work, counseling, or a related behavioral health field, or a degree in nursing, combined with at least three years of clinical experience in social work counseling involving complex psychiatric and substance abuse or substance use disorder treatment; an equivalent combination of education and experience may be accepted.
  • An active, current, and valid license as a Registered Nurse, Licensed Clinical Social Worker, Licensed Mental Health Counselor, Licensed Professional Counselor, Licensed Marriage and Family Therapist, or Clinical Psychologist, as applicable under state law and scope of practice, to practice as a health professional in applicable states or territories of the United States.

  • Associate-level licenses, including Licensed Clinical Social Worker-Associate and Licensed Clinical Mental Health Counselor-Associate, cannot be accommodated.
Responsibilities
  • Conduct assessments to identify individual needs.
  • Develop comprehensive care plans addressing objectives and goals identified during assessment.
  • Support member access to appropriate, quality, and cost-effective care and modify care plans as needed.
  • Coordinate with internal and external resources to meet members' identified needs through integrated physical and behavioral health and whole-person care.
  • Coordinate social determinants of health services to meet members' needs and incorporate them into care planning.
  • Work closely with state agencies and maintain knowledge of the system-of-care philosophy and coordinated networks of effective community-based services and supports for people with, or at risk for, mental health and other challenges and their families.
  • Build meaningful partnerships with designated populations and their families and address cultural and linguistic needs to support functioning at home, in the community, and throughout life.
  • Evaluate health needs and identify applicable services and resources in conjunction with members and their families.
  • Provide patient education, perform medication reconciliation, identify community resources, and assist with arranging follow-up care.
  • Perform case management telephonically and/or through home visits within the scope of licensure for special programs such as foster care.
  • Manage overall healthcare costs for the designated population through integrated physical and behavioral health case management and whole-person health.

  • Clinical experience in social work counseling with a broad range of complex psychiatric, substance abuse, and/or medical disorders.
  • Ability to travel within the assigned Department of Social Services region.
