Requirements
  • Have excellent communication skills in order to serve as a vital link between the nursing staff, various departments, residents, and visitors.
  • Ability to direct and perform a variety of activities and to respond to varied needs of the residents.
Responsibilities
  • Oversee all aspects of the Activity Program.
  • Coordinate pre-admission assessments and screenings.
  • Develop care plans including identifying behavior triggers, appropriate responses and interventions, and individual resident preferences.
  • Create and maintain a comfortable and homelike environment on the Memory Care unit.
  • Schedule staff and activities.
  • Oversee resident eligibility for the Memory Care program and maintain appropriate residents on the unit.
  • Maintain all safety requirements for the Memory Care program and unit.
  • Provide orientation and training for all employees to work on the Memory Care unit.
  • Coordinate the discharge process.
  • Lead continuous quality improvement processes on the unit.
  • Utilize the electronic medical record system and monitor for compliance.
  • Maintain resident charts in accordance with policy and procedures.
  • Serve as the primary liaison to the community, families, and area hospitals.
  • Be responsible for ongoing training and learning in the dementia field.
  • Train staff to keep all staff training current.
  • Maintain effective communication with the Diet Department to enhance the nutrition program for residents.
  • Maintain effective communication with therapy services to enhance quality of care and quality of life.
  • Coordinate monthly family support meetings.
  • Serve as a model for positive relationships on the Memory Care unit.
  • Meet and exceed all regulatory requirements established in this state.
Desired Qualifications
  • Preferred RN licensure in nursing.
  • Experience working with residents with dementia diagnosis and understanding of disease process, stages of dementia and programming needs of dementia population.
  • Imagination and creativity preferred.
