Requirements
  • A Bachelor's degree in nursing or a closely related field is required.
  • Current Maryland licensure as a Registered Nurse is required.
  • Annual certification in cardiopulmonary resuscitation is required.
  • At least five years of experience in case management is required.
  • At least one year of team leader or other leadership experience is required.
  • Knowledge of insurance benefit structures, Medicare Conditions of Participation, and applicable regulations is required.
  • Knowledge of utilization management and quality improvement processes is required.
  • Knowledge of outpatient services and settings is required.
  • Completion of the Training Checklist for Managers and Supervisors within twelve months of employment or promotion is required.
Responsibilities
  • Ensure adequate staffing coverage by monitoring staffing needs, preparing schedules, approving leave requests, and arranging coverage for absent staff.
  • Keep policies and procedures current and educate and monitor staff for compliance.
  • Coordinate interdisciplinary care for a select caseload of patients from pre-admission through post-discharge when assigned.
  • Coach case managers through review and ongoing case discussion to develop safe transition plans that account for patient and family preferences.
  • Evaluate readmissions with the Collaborative Practice Team to identify improvements to transition plans.
  • Attend interdisciplinary rounds to identify barriers in transition planning and assist case managers in addressing them.
  • Ensure interdisciplinary rounds follow standard work processes and collaborative communication practices.
  • Develop relationships with medical staff to increase collaboration and ensure reliable communication about initiatives and patient care issues.
  • Act as a preceptor for case managers during onboarding or oversee orientation when delegated.
  • Incorporate reimbursement principles and managed care strategies into case management services.
  • Participate on relevant administrative and clinical committees.
  • Ensure staff are oriented to assigned areas and maintain required training and continuing education.
  • Identify undesirable trends in clinical service delivery and patient outcomes and participate in performance-improvement efforts.
  • Manage personnel activities including performance reviews, counseling, discipline, and support for resolving complex employee issues.
  • Monitor the quality of services rendered by staff, evaluate monitoring results, and implement corrective actions for deviations from standards.
  • Manage fiscal resources by participating in annual budget preparation, monitoring expenditures, and completing required reports.
  • Participate in leadership activities and special projects assigned by the Director.
  • Review and authorize charity care.
Desired Qualifications
  • A Master's degree is preferred.
  • Certification as an ACM or CCM is required within two years of the position.
