Requirements
  • A bachelor's degree in Social Work, Sociology, Psychology, or a related field, or a Registered Professional Nurse currently licensed to practice in the State of Georgia.
  • Two years of experience in the human service or health-related field plus one year of experience as a case manager.
  • Computer skills proficient enough to master the WellSky database system.
  • Ability to work independently and as part of a team.
  • Ability to communicate with clients having a variety of educational levels.
  • A valid state driver's license.
  • Eligibility to take the CIRS-A/D examination after one year of experience.
  • Eligibility to take Boston University courses after becoming CIRS-A/D certified and completing one year of experience.
  • Ability to meet the physical and mental demands required to perform the essential functions, with reasonable accommodation available where applicable.
Responsibilities
  • Receive and screen emergency and non-emergency referrals by telephone.
  • Document potential referrals on the screening and assessment instrument in the state database.
  • Review initial financial, medical, and social information of potential clients as presented by the referral source or applicant.
  • Verify Medicaid eligibility and screen for MAO/PMAO eligibility using standardized financial guidelines.
  • Explain the scope and purpose of CCSP and HCBS.
  • Identify client needs and desired services as stated by the referral source or applicant.
  • Screen clients for HCBS, CCSP, and other respite programs.
  • Determine when a client is ineligible or inappropriate for CCSP placement and refer the client to appropriate resources.
  • Determine when a client is eligible and appropriate for CCSP placement and refer the client for a full CCSP assessment; place the client on a waiting list when CCSP is not admitting new referrals.
  • Determine client eligibility for HCBS programs, place clients on a waiting list, or refer them to appropriate resources.
  • Follow HIPAA guidelines.
  • Assist with developing and revising policies and procedures.
  • Attend organizational meetings and required training.
  • Keep the Director and Lead informed about intake and assessment problems and progress.
  • Meet department productivity goals.
  • Refer clients on the waiting list to community resources for assistance.
  • Maintain required standards of promptness for all programs.
  • Complete appropriate documentation and paperwork for all programs.
  • Complete full rescreens for clients on CCSP and HCBS waiting lists when required.
  • Provide information and assistance to callers, as well as options counseling, with follow-up as required.
  • Travel locally and attend off-premises meetings, conferences, or training as required.
