Requirements
  • A high school diploma or equivalent is required.
  • A valid New York, New Jersey, Connecticut, or Pennsylvania driver's license with at least six months of experience and six or fewer points is required.
  • Strong interpersonal and communication skills, patience, empathy, and a genuine desire to support people with developmental disabilities are required.
  • The ability to respond effectively to changing needs and environments, demonstrating flexibility and problem-solving skills, is required.
  • Employees must maintain confidentiality, uphold ethical standards, and follow agency policies and procedures.
  • A strong commitment to safety is required.
Responsibilities
  • Implement and monitor varying plans of care for the people supported, including Comprehensive Functional Assessments, Staff Action Plans, Life Plans, treatment plans, and Behavior Support Plans.
  • Implement and document goals and objectives from person-centered plans, focusing on social, vocational, and life skills through structured activities and positive reinforcement.
  • Provide direct care and assistance with daily living activities, including personal hygiene, meal preparation, medication administration, and transportation.
  • Facilitate community outings, recreational activities, and social events to promote inclusion, independence, and meaningful interactions.
  • Accompany people supported to appointments and healthcare follow-ups.
  • Use person-centered approaches and positive behavior support strategies to address challenging behaviors and provide appropriate supervision and support.
  • Maintain accurate records, daily progress notes, and incident reports covering achievements, behaviors, and health status in compliance with agency policies and regulatory requirements.
  • Collaborate with healthcare professionals, therapists, and family members to develop and implement comprehensive care plans.
  • Advocate for the rights, choices, and preferences of individuals with developmental disabilities, promoting self-advocacy and empowerment in decision-making.
  • Serve as a custodian and mandated reporter in accordance with OPWDD, NYS Justice Center, and organizational policies.
  • Administer medications according to prescribed schedules, document medication administration, and monitor for adverse reactions or side effects.
Desired Qualifications
  • Experience in direct care or human services is preferred.
  • CPR and First Aid certification are preferred.
  • Additional training in developmental disabilities, behavior management, or healthcare is desirable.
