Requirements
  • A bachelor's degree in human services, psychology, sociology, social work, or a related healthcare field is required.
  • Complete Basic Life Support cardiopulmonary resuscitation training assigned by Education.
  • Complete ACMA Compass training modules for new employees assigned by the Director of Case Management.
Responsibilities
  • Coordinate and facilitate patient care across a multidisciplinary care continuum within a clinical service unit structure.
  • Ensure optimum utilization of resources, service delivery, and compliance with clinical cost education initiatives.
  • Advocate for patients and manage resources to facilitate patient-centered solutions and the best possible patient outcomes.
  • Maintain appropriate documentation.
  • Assess and evaluate patients' ability to provide self-care, and develop discharge plans and goals of care with patients and involved caregivers.
  • Assist with making referrals to related agencies while taking freedom of choice into consideration.
  • Provide information to social service and discharge planners to facilitate the discharge planning process.
  • Participate in multidisciplinary rounding and care conferences with other departments.
  • Serve as a resource person to physicians and hospital staff regarding the appropriate use of health care resources.
  • Serve as a liaison with nursing and medical staff and community agencies to facilitate problem solving and coordinate services.
  • Assess patients and develop a plan early upon admission, revising it as needed according to patient needs.
  • Accept on-call assignments and respond as needed.
  • Take responsibility for upgrading professional skills and knowledge.
  • Prepare and complete reports and participate in special projects.
  • Demonstrate clinical competency by completing assigned training modules.
  • Perform other duties as assigned.
