Requirements
  • Ability to maintain all required continuing education and licensing and remain in good standing with the State Board of Nursing.
  • Current, unencumbered, active license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse in Pennsylvania.
  • Current CPR certification.
  • Nursing degree from an accredited college or university, or graduation from an approved Licensed Practical Nurse/Licensed Vocational Nurse program.

Responsibilities
  • Provide direct nursing care to residents and supervise the day-to-day nursing activities performed by nursing assistants.
  • Direct the day-to-day functions of nursing assistants in accordance with applicable rules, regulations, and guidelines.
  • Ensure assigned nursing personnel comply with facility policies, procedures, job descriptions, and the Nursing Service Procedures Manual.
  • Plan shift services, programs, and activities with assigned nursing and support personnel.
  • Review nursing policies, procedures, manuals, and job descriptions and recommend revisions.
  • Prepare written and oral reports and recommendations concerning shift activities.
  • Coordinate nursing services with other resident services to maintain each resident’s total care regimen.
  • Participate in the nursing service quality assurance program and facility surveys.
  • Review and update resident discharge plans and assist with planning nursing services for discharge.
  • Interpret department policies and procedures for personnel, residents, visitors, and government agencies.
  • Admit, transfer, and discharge residents and complete related forms and reports.
  • Complete accident and incident reports and maintain required census, charge, medical, and administrative records.
  • Protect resident health information and facility information-system credentials and report suspected unauthorized access.
  • Complete admission, transfer, discharge, charting, and documentation records.
  • Receive and record physician telephone orders and transcribe orders to resident charts, medication cards, treatment plans, and care plans.
  • Chart nurses’ notes and all accident or incident reports accurately and sign and date medical-record entries.
  • Record diet orders and report discrepancies in physician orders, diet changes, or charting.
  • Prepare and administer medications and treatments as ordered by physicians.
  • Verify resident identity and prevent medication administration errors.
  • Maintain adequate medication, supply, and equipment stocks and order items as needed.
  • Maintain accurate narcotic records, report discrepancies, review medication cards, process automatic stop orders, and dispose of drugs and narcotics according to procedure.
  • Participate in employee performance evaluations, determine shift staffing requirements, and make staffing recommendations.
  • Report staffing shortages and absentee call-ins to the Nurse Supervisor.
  • Develop work assignments and provide leadership to nursing personnel assigned to the unit or shift.
  • Make daily rounds to verify that nursing personnel perform assignments according to nursing standards.
  • Ensure nursing assistants meet training or supervision requirements.
  • Meet regularly with shift personnel to identify and correct problems and improve services.
  • Enforce departmental policies and procedures and review personnel complaints and grievances.
  • Receive and give nursing reports at shift changes.
  • Report occupational exposures, suspected fraud, and information-security violations.
  • Inform nursing personnel about new admissions and prepare rooms for incoming residents.
  • Greet and orient newly admitted residents and family members.
  • Make physician rounds and arrange ordered diagnostic and therapeutic services.
  • Consult physicians regarding resident care, treatment, and rehabilitation.
  • Review resident charts for treatments, medication orders, diets, and other care instructions.
  • Implement and maintain nursing objectives and standards.
  • Check that prescribed treatments are administered correctly and evaluate residents’ physical and emotional status.
  • Coordinate social and activity programs with nursing schedules.
  • Notify physicians and next of kin about accidents, incidents, or changes in resident condition.
  • Carry out restorative and rehabilitative programs, including self-help and care.
  • Maintain clean and safe treatment areas.
  • Perform professional nursing services including catheterization, tube feeding, suctioning, dressing changes, colostomy and drainage-bag care, blood collection, massage, range-of-motion exercises, and care of dying residents.
  • Use restraints according to established policies and procedures.
  • Obtain ordered sputum, urine, and other specimens for laboratory testing.
  • Take and record temperature, pulse, respirations, blood pressure, and other measurements.
  • Monitor seriously ill residents.
  • Check food brought by visitors against residents’ dietary allowances.
  • Ensure direct care follows resident care plans and wishes and that residents unable to call for help are checked frequently.
  • Meet with residents and family members and report problems to the Nurse Supervisor.
  • Arrange transportation for discharged residents and escort residents to the pick-up area.
  • Inform family members of resident deaths, contact funeral homes when requested, and follow post-mortem procedures.
  • Develop, plan, conduct, and schedule in-service training.
  • Orient new employees to the shift, policies, procedures, and job duties.
  • Assist with standardizing work methods and training personnel on blood and body-fluid exposure risks.
  • Assist the Director with clinical supervision planning for nurse aide trainees.
  • Attend external and annual facility in-service training, advance-directive training, and continuing education.
  • Monitor personnel compliance with safety regulations, dress codes, hand hygiene, infection control, and equipment procedures.
  • Maintain personal protective equipment and safety supplies and report hazardous conditions, missing labels, or missing safety data sheets.
  • Participate in infection-control and communicable-disease monitoring programs.
  • Maintain clean, safe, and sanitary nursing areas and resident-care rooms.
  • Recommend unit equipment and supply needs and maintain adequate stock levels.
  • Ensure trained and authorized personnel operate nursing equipment safely and supplies are used efficiently.
  • Maintain safety data sheets for hazardous chemicals.
  • Review care plans daily, identify needed changes, and ensure documentation reflects care-plan compliance.
  • Ensure certified nursing assistants understand and follow resident care plans.
  • Assist with the Resident Assessment/Care Plan Coordinator’s Minimum Data Set planning, scheduling, and revisions, including implementation of RAPs and triggers.
