Requirements
Must possess an active, unencumbered license to practice as a Nurses Practitioner in Maine and be knowledgeable of nursing and medical practices and procedures.
Must be eligible to practice independently as outlined by the Maine Department of Professional and Financial Regulation Board of Nursing.
Must be licensed to practice in the outpatient primary care setting.
A current valid license to operate a vehicle in the state of Maine and a driving record that is satisfactory to the company may be required.
Frequent travel is required to service residents across the state in their residence at a Woodlands Senior Living community.
Must complete a criminal history record check to the satisfaction of the company.
Responsibilities
Obtains patient history and performs comprehensive assessment via observation, interview, and examination.
Develops individualized care plans to manage conditions and collaborates with multidisciplinary teams to ensure the best possible outcomes for the resident.
Provides direct treatment and management of health conditions, including use of prescriptive authority and referral to other healthcare providers and community resources.
Orders, performs, and interprets diagnostic studies.
Provides counseling and education to residents and their families concerning preventative health, treatment options and community resources.
Provides care in the context of promoting age and culture, appropriate informed choices and shared decision-making.
Desired Qualifications
Demonstrated experience as an independent Nurse Practitioner is preferred.
