Requirements
  • A Diploma in Nursing is required.
  • A valid and active Licensed Practical Nurse license in Kansas and Missouri, or a multi-state compact license, is required.
  • A current Basic Life Support certification from the American Red Cross or American Heart Association is required.
  • Licenses and certifications must be maintained throughout employment.
  • Intermediate knowledge or experience with common diagnoses typically seen in primary care is required.
  • Ability to work effectively in a fast-paced environment with competing priorities is required.
  • Ability to define technical and clinical problems, collect data, establish facts, and draw valid conclusions and action plans is required.
  • Intermediate knowledge of Microsoft Windows applications is required.
  • Ability to use member tools and applications to access and document health information and support member interactions through phone, in-person, chat, and email channels is required.
  • Ability to comply with Spira Care's vaccination requirements, with or without reasonable accommodation, is required.
Responsibilities
  • Provide professional nursing care for care center patients following established standards and practices.
  • Establish a trusted partnership with members through healthcare knowledge sharing and education, using critical thinking and prioritization in collaboration with the care team.
  • Provide customer service through incoming member calls, return calls, triage, in-person interactions, and member outreach.
  • Administer and document member intake, point-of-care testing, immunizations, injections, and oral medications; assist providers with procedures; and perform phlebotomy as needed.
  • Assist with care coordination, including pre-visit planning, clinical pathway adherence, care gap identification and management, emergency room and inpatient discharges, and transitions of care.
  • Serve as a liaison for members' healthcare needs and use communication channels to identify needs for Behavioral Health, Health Coaching, Diabetic Educators, and Blue Cross and Blue Shield programs.
  • Document and complete tasks assigned to electronic health record inboxes and member call queues.
  • Work with the care team to meet quality performance targets and provider and care center metrics.
  • Maintain confidentiality of regulated information in compliance with state and federal laws and follow medical, corporate, divisional, and departmental policies and procedures.
  • Travel locally as required.
Desired Qualifications
  • Prior primary care experience is preferred.
  • Experience with Athena electronic health record is preferred.
  • Experience with Affordable Care Act and/or Medicare populations is preferred.
  • Advanced knowledge or experience with common diagnoses typically seen in primary care is preferred.
