Requirements
  • Hold a current Registered Nurse license.
  • Graduate from an approved school of nursing.
  • Have a minimum of two years of experience in an acute setting requiring heart, lung, and physical assessments, or demonstrate equivalent ability or education in these areas.
  • Maintain current CPR certification.
  • Function with minimal supervision.
  • Manage multiple projects and changing priorities.
  • Employ critical-thinking skills.
Responsibilities
  • Complete initial and periodic comprehensive assessments of clients, caregivers, and home environments for admission and continued agency service, including appropriate OASIS items, to determine care needs, effectiveness, and achievement of anticipated outcomes.
  • Accept client admissions assigned by the agency and weekend on-call assignments.
  • Provide comprehensive nursing care using home assessments, nursing diagnoses, and physician-approved care plans.
  • Perform specialized treatments ordered by the physician.
  • Observe signs and symptoms and report them to the Vice President of Clinical Services, Director of Professional Services, and/or physician as appropriate.
  • Note changes in clients' physical or emotional status.
  • Assess clients' nutritional needs and diets as ordered by the physician.
  • Educate patients and families about medications and procedures.
