- At least 5 years of recent clinical experience in a physician’s office, clinic, hospital, or emergency services (EMS) setting, or other equivalent experience.
- Master’s Degree in Social Work from CSWE accredited program.
- Be responsible for exemplary cross continuum care coordination, inclusive of psychosocial assessments and interventions.
- Work with physicians, peers, community colleagues, and others to orchestrate care across the health care continuum.
- Identify opportunities to continually improve patient care and services.
- Improve population health.
- Achieve collaborative practices that exemplify Main Line Health System’s commitment to patient centered care and community engagement.
- Leadership experience a plus.
- Licensed Social Worker strongly preferred.
Lankenau Institute for Medical Research (LIMR) is the research division of Main Line Health that conducts cancer and biomedical research and translates basic science into clinical applications. LIMR studies cell growth and cancer biology, develops discoveries in the lab, and collaborates to move findings into patient care within the health system. It differs from other research centers by its long history as one of the first U.S. cancer-focused institutes, its integration within a hospital-based health system, and its track record of translational research—turning lab findings into clinical therapies and practices. The institute aims to improve patient outcomes by turning basic science into usable medical treatments and interventions for cancer and related diseases.
