Responsibilities
  • Provides individual, supportive coaching to improve physical health status for assigned clients.
  • Coordinates client care with team of internal/external providers.
  • Provides life skills training, care management, and structured interventions to assist consumers in gaining access to resources and the skills necessary to improve their physical health status, quality of life, and self-management of symptoms.
  • Provides support across the phases of care that is responsive to the changing needs of the persons served.
  • Understands how the job functions within the overall Certified Community Behavioral Health Clinics (CCBHC) model.
  • Perform all required job duties and responsibilities in accordance to CCBHC Standards.
Desired Qualifications
  • BA/BS from a four-year college/university preferred.
