Licensed Vocational Nurse Supervisor

Overview

We are recruiting for a PACE (LVN) Supervisor to join our growing team.

This exempt position is a crucial part of the service delivery team responsible for quality participant care at the PACE center. This position supervises and directs health center staff activities to ensure quality of care, patient flow, and compliance with regulations and governing bodies. Additionally, a member of the management team, the Licensed Vocational Nurse (LVN) Supervisor evaluates coaches and guides staff performance along with providing indirect and direct patient care within the scope of their respective nursing license.

Responsibilities

PERFORMANCE AREA 1: Clinic Management and Supervision

• Receives and communicates appropriate information to staff and keeps PACE Director informed of changes, needs, complaints, etc.

• Responds to emergencies involving participants.

• Participates in the Quality Assurance program by conducting audits and submitting results in writing.

• Directs, supervises and evaluates the performance of the clinic team, and maintains a matrix reporting/supervisory relationship with staff involved in participant care.

• Interviews, recommends for hire, and trains staff specifically for Clinic Operations.

• Prepares periodic reports and assists with QI projects to review staff performance as related to planning, quality of work, procedures, and participant care.

• In collaboration with other site managers and supervisors, establishes and maintains uniform, consistent delivery of care and services for participants, including internal and external appointments with transportation needs.

• Arranges Licensed Vocational Nursing (LVN) and Medical Assistant (MA) coverage for all areas of responsibility.

• Works with supervisory staff and site director to assure that cultural needs of participants are met in the day center.

• Assists in the development and implementation of new procedures.

• Teaches participants and families based on identified needs of participant.

• Able to triage participants in clinic effectively.

• On call responsibilities.

• Participates and functions as a member of the interdisciplinary team and rotates as needed.

• Schedules and coordinates med pass and monitor for quality review of medication errors.

• Mentors and assists in the orientation of new clinic staff hires.

• In collaboration with the purchasing agent, oversees medical, pharmaceutical and oxygen supplies in the clinic. Tracks assignment of oxygen concentrators and other Durable Medical Equipment as indicated.

• Scheduling and managing appointments/assessments for MDs and Nurse Practitioners

• Wound care and maintaining wound records/documentation

• Hold formal monthly and as needed meetings with staff and records minutes.

• Monitor transportation services and resolves problems as necessary with Transportation Coordinator.

• Consult and collaborate with the Dietitian and Recreation Therapist on matters related to food/nutritional services within the Adult Day Health Center.

• In consultation with other management staff, develop and implement policies, procedures and standards for Health Center. Communicate changes to staff, monitor outcomes and makes changes as needed.

• Assume a leadership role in the development, implementation and refinement of day center systems and coordination of schedules which involves rigorous communication and collaboration with all departments, including transportation, the clinic, and rehabilitation therapy, recreation therapy, dining services, home care, social work, maintenance and environmental services.

• Interface with program participants, families, contracted providers and the public in regard to issues related to the routine operation of the Health Center.

• Monitor the physical environment of the Health Center in conjunction with assigned staff, to ensure a safe, orderly, and sanitary environment is consistently maintained for participants and staff. Notifies appropriate maintenance or housekeeping staff of problems or concerns as indicated; consults with PACE Director regarding significant repair/replacement needs.

• Review safety issues, including participant falls and incidents as they occur, and reports to appropriate personnel, and appropriate committees.

• Serve on assigned committees of the organization and regularly attends meetings of same.          

• Assist the PACE Director with budgeting, monitoring and analyzing site performance trends in utilization, financial and participant outcomes. 

• Collaborate with PACE Director to investigate and resolve grievances, service delivery concerns, and incidents relating to Health Center operations.

• Supervise the Health Center/Day Center staff.

• Act as liaison between providers/family/pharmacy and will be an active member of interdisciplinary team (IDT).

• Supports the health centers cooperative approach to team building by supporting a team approach to site leadership. Must be able to demonstrate exceptional written, verbal, and interpersonal communication skills and be able to clearly communicate information.

• Improves patient access by participating with the PACE Director and site leadership in the analysis of clinical systems and in the development, implementation and on-going evaluation of clinic flow process; this includes actively participating in corporate and site-specific performance improvement activities.

• Supports the health center financial responsibility by ensuring the maintenance of site inventory and ordering of medical supplies meets the participant need and corporate fiscal needs.

• Responsible for the on-going development and implementation of UHC's Safety Plan, using processes and activities to reduce and control environmental hazards and risks, prevent accidents and injuries, and maintain safe conditions for patients, visitors, and staff.

• Participates in annual budget development related to areas of responsibility.

• Supports the continued development of efficiencies by working closely with site leadership in the analysis of site budgets, costs, and the implementation of cost containment measures.

PERFORMANCE AREA 2: Nursing Duties

• Supports participant access by screening participants and routes appropriately.

• Meets regulatory and continuity of care by utilizing standing orders and established screening guides under supervision of physicians.

• Documents nursing care in the medical record.

• Supports the internal and external participant care by taking responsibility for outpatient medical chart using SOAP method of charting.

• Performs functions related to clinical treatment, care planning, oversight of participant care.

• Supports participant access by providing telephone advice regarding care per health center protocol.

• Supports the efforts of the providers by supervising the functions in treatment room and is primary in this regard.

• Supports participant care by assisting the physician and/or provider in procedures and pelvic exams

• Supports participant care by administering medication and treatment ordered by physicians and/or provider staff.

• Completes transcription and follow-up of medical orders.

• Performs phlebotomy procedures as needed.

• Maintains cleanliness and organization in the clinic, especially in the medication area and cart.

• Maintains and oversees all logs related the health center regulatory requirements.

• Daily nursing tasks, preparing clinic for patients to be seen by provider.

• Assist with bedside procedure including gynecological exams, phlebotomy.

• Maintaining participant safety while at the center

PERFORMANCE AREA 3: General Corporate Expectations

• Attends and actively participates in all meetings (e.g., department meetings, program meetings, employee staff meetings) and other activities as required or assigned.
• Attends workshops/seminars as necessary to increase skills and knowledge to provide effective care, treatment, and/or leadership.
• Supports the overall needs of the health center by working flexible or extended hours when necessary.
• Supports the needs of the health center by traveling to other UHC clinics when staffing needs dictate the need for licensed personnel.
• Demonstrates awareness of, and compliance with organizational mission and objective of UHC to provide health care access and support services for all members of the community.
• Supports their own staff development by completing the required hours of continuing education each year as well as new certifications as required.
• Other work-related duties as assigned by supervisor. Duties and responsibilities may be added, deleted, or changed at any time at the discretion of management, formally or informally either verbally or in writing.

Qualifications

EDUCATION AND LICENSE/CERTIFICATION:

• High School Diploma or GED Certificate 

• Graduate of an accredited school and possess a valid Licensed Vocational Nurse (LVN)License

• Valid Advanced Cardiovascular Life Support (ACLS) Certification

• Valid IV Certification

PRIOR EXPERIENCE:

• Minimum 2 years of nursing experience, preferably in an ambulatory care setting

• Minimum 1 years of management or supervisory experience preferred

• Experience with facility and program licensure and audits a plus

The pay range for this Exempt position starts at $73,696.94 a year. Our salary ranges are dependent on knowledge, skills, and experience.
