Requirements
  • A minimum of a Bachelor's Degree in human services or a related field is required.
  • Two years of experience working with youth or families is required.
  • Family Centered Treatment Certification must be completed within one year of employment to continue providing Family Centered Treatment services and maintain employment.
  • Complete Family Centered Treatment checkoffs and other internal trainings within designated deadlines.
  • Be provisionally credentialed as a Provider of Community Support Services in Mississippi and eligible to be fully credentialed as a Community Certified Support Specialist in Mississippi, where applicable.
  • Meet the qualifications for a Qualified Professional in North Carolina, including experience serving children and adolescents with a primary mental health diagnosis, where applicable.
Responsibilities
  • Provide Family Centered Treatment services according to the service definition and model fidelity requirements.
  • Deliver Family Centered Treatment services to children, adolescents, and their families primarily in their living environments, with a family focus.
  • Provide 24/7 on-call coverage to a caseload of families and children, including services in homes and communities according to family needs.
  • Complete required clinical records and Family Centered Treatment documents with the child and family and enter or upload them timely in the electronic health record.
  • Develop the service plan in coordination with the treatment team and maintain it according to designated authorization processes.
  • Collaborate with treatment team members to ensure effective communication and coordinated service provision.
  • Revise and implement the service plan on an ongoing basis.
  • Provide psychoeducation as indicated in the service plan.
  • Conduct family interventions aligned with the Family Centered Treatment model, facilitate movement through the four treatment phases, and complete fidelity documents.
  • Assist families and children in establishing weekly goals and identifying steps to reach those goals.
  • Make referrals to services and resources for families and children.
  • Develop and coordinate discharge plans based on treatment, including relevant aftercare needs and plans.
  • Access community resources beneficial to client aftercare needs.
  • Coordinate and oversee initial and ongoing assessment activities.
  • Convene the treatment team for service planning.
  • Consult with medical and non-medical providers, engage community and natural supports, and incorporate their input into person-centered planning.
  • Ensure linkage to clinically appropriate services, including arranging psychological and psychiatric evaluations.
  • Provide and coordinate behavioral health services and interventions with behavioral health professionals and treatment team members.
  • Monitor and document consumer progress.
  • Participate in quality assurance and quality improvement activities, including audits, corrections, improvement plans, and designated projects.
Desired Qualifications
  • Current Family Centered Treatment Certification is preferred.
  • A current Qualified Mental Health Professional certification or ability to register as a Qualified Mental Health Professional with the Virginia Board of Counseling is preferred.
  • A Licensed Social Worker license or eligibility for provisional licensure is preferred.
