Requirements
  • A High School Diploma or GED is required.
  • At least one year of experience providing direct care services to adults with a developmental disability is required.
  • AMAP certification, SCIP-R training, and CPR and First Aid training are required.
  • A valid New York State driver's license is required, and the candidate must be able to pass the insurance criteria.
  • Candidates must be able to support people with intellectual and developmental disabilities in a safe environment.
Responsibilities
  • Assist each consumer in skill development in areas such as independent living, adaptive behaviors, socialization, recreation, and activities of daily living.
  • Supervise consumers in the community, using various experiences to reinforce skills taught in group sessions while ensuring their safety.
  • Lead group sessions with consumers and facilitate discussions to provide learning skills for use in their community.
  • Provide annual and semi-annual progress reports regarding valued outcomes.
  • Assess the needs of lower-functioning and/or non-verbal consumers through observations and evaluations, and develop and address their valued outcomes.
  • Provide input and ensure compliance with the valued outcomes in the Individualized Service Plan and Day Habilitation Plan.
  • Provide weekly documentation of outcomes being worked toward with each individual in accordance with their Day Habilitation Plan.

  • An associate degree is preferred.
