Requirements
  • A New York State Licensed Practical Nurse license is required.
  • Experience working on a multidisciplinary team and/or prior experience with the homeless population is required.
Responsibilities
  • Meet with the Program Director, Director of Social Services, Registered Nurse, and Social Service staff to coordinate shelter residents' ongoing treatment in the community.
  • Take vital signs, including blood pressure, temperature, and weight.
  • Assist with diabetes medication and treatment.
  • Take medical histories and enter information into computer systems.
  • Coordinate with Case Managers, Senior Case Managers, and the Director of Social Services to ensure proper care, nutrition, and treatment plans are followed.
  • Follow up with community health care providers to ensure medical treatment plans are documented, monitored, and followed.
  • Work a schedule that includes evenings and weekends; the listed schedule is Tuesday through Saturday, 9:00 a.m. to 5:00 p.m. or 10:00 a.m. to 6:00 p.m., with one flexible late-shift day.
Desired Qualifications
  • Bilingual English/Spanish ability is preferred but not required.
