Responsibilities
  • Collaborate with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to patients in their homes, including conducting patient home visits.
  • Assess, plan, direct, and evaluate each patient's care plan to ensure it meets the patient's hospice needs.
  • Manage emergency situations promptly, precisely, and professionally.
  • Counsel patients and their families regarding end-of-life needs.
  • Complete medication administration and treatment needs for assigned hospice patients.
  • Provide nursing care in accordance with applicable regulations and company policy.
  • Prepare and maintain accurate and complete clinical and progress notes on a timely basis.
