- Effective communication, comprehension, documentation, interpersonal, and computer skills.
- Ability to assess patient needs and formulate individualized patient care plans.
- Ability to work in a field setting and make sound judgments.
- Master's degree in social work from a school of social work accredited by the Council on Social Work Education.
- Valid driver's license and reliable transportation insured in accordance with Ohio requirements.
- Active Ohio social work license.
- Minimum of 1 year of experience in a health care field, including experience working with patients and families coping with terminal illness and dying.
- Conduct an initial comprehensive psychosocial and bereavement risk assessment of the patient, family, and caregiver within five calendar days after the patient's election of hospice care.
- Obtain and make referrals to community resources, including financial assistance and community agencies, with appropriate follow-up.
- Assist families and caregivers with placement of patients in facilities for basic care, respite, or inpatient care when appropriate.
- Provide psychosocial and anticipatory grief counseling to patients, families, caregivers, and significant others regarding the impact and implications of terminal disease and patient care.
- Assess the psychosocial status of patients and families in relation to coping capacity, terminal illness, environment, and death; provide counseling, assist with community resources, and communicate findings to the team.
- Participate in developing plans of care and treatment and hold case conferences as needed.
- Prepare social histories and evaluations and plan interventions based on findings.
- Update, maintain, and submit accurate and timely clinical documentation according to agency guidelines and assist with discharge planning.
- Attend scheduled interdisciplinary team meetings to coordinate care plans, follow up on changes, problem-solve, identify social problems and their relationship to medical situations, assess family strengths and weaknesses, and discuss alternative interventions.
- Assist team members in understanding significant social and emotional factors affecting patients and families.
- Provide care respectfully and with dignity, and report complaints, abuse allegations, misappropriation of patient property, and other corporate compliance matters.
- Teach patients, families, and caregivers about disease progression and self-care techniques, including counseling support and instructions ordered by the physician.
- Hospice or home care experience is preferred but not required.
- Act as a mentor to other personnel by participating in orientation and assisting with field training and in-service education for contractors and referral sources.
