Requirements
- An active Pennsylvania Licensed Practical Nurse (LPN) license is required.
- The position requires working from 2:30 PM to 11:00 PM, every other weekend, and holidays.
Responsibilities
- Administer treatments and medication as specified in the medication administration record, treatment administration record, and as-needed medication sheets.
- Assemble and use equipment including catheters, tracheotomy tubes, and oxygen supplies.
- Assist residents with activities of daily living as needed.
- Review the preceding shift’s 24-hour report and general notes in the resident care system.
- Complete unfinished orders and tasks from the preceding shift.
- Ensure assigned resident care is completed.
- Document changes in residents’ care and condition.
- Maintain residents’ appointment calendars and ensure transportation to appointments.
- Assess residents’ conditions and inform physicians and families or responsible parties of changes and the treatment plan as needed.
- Receive physician orders, record the time, date, and initials, and transfer orders to the resident care system.
- Notify the pharmacy of medication-order changes or refill needs, and initial and date order changes.
- Comply with Pennsylvania Department of Human Services regulations and Willow Valley Communities policies and procedures.
- Complete laboratory requests with all pertinent information according to physician orders.
- Ensure residents receive annual physician evaluations or evaluations when their condition changes, and ensure the physician completes the required DME.
- Ensure a RASP is completed with each DME.
- Ensure physicians complete necessary paperwork.
