  • A bachelor's degree in Nursing (BSN) is required.
  • Current New York State Registered Nurse licensure is required.
  • Three to five years of healthcare experience in an acute care setting is required.
  • Superior communication, demonstrated problem-solving skills, strong business acumen, and interpersonal skills are required.
Responsibilities
  • Manage complex cases and serve as a liaison between acute care and critical care teams.
  • Coordinate the efforts of social workers, feeding specialists, physical therapists, occupational therapists, cardiologists, intensivists, and neonatologists in support of patients and their families.
  • Direct all aspects of inpatient care and ensure optimal length of stay.
  • Anticipate outpatient service needs and set appointments and consultations before discharge.
Desired Qualifications
  • Two years of case management or care coordination experience in an inpatient, HMO, insurance, or other outpatient setting.
  • One year of leadership experience in a healthcare setting.
  • A master's degree in a healthcare-related field.
