Requirements
  • A bachelor's degree and Master of Social Work (MSW) or an MSW-equivalent program.
  • An active Licensed Independent Clinical Social Work (LICSW) license.
  • At least two years of clinical work experience.
  • Ability to use Cognitive Behavioral Therapy (CBT) or similar techniques.
  • Working knowledge of medical terminology.
  • Licensure, or willingness to obtain licensure, in Rhode Island, Massachusetts, Connecticut, and other applicable states.
  • Comfort using Slack, Zoom, Google Suite, and electronic health record systems.
  • Ability to establish rapport and work effectively with health care providers, clinical staff, patients, and family members.
  • Ability to identify and prioritize initiative needs and act on urgent and important needs.
  • Comfort working with limited direction and ambiguity.
  • Strong organization, attention to detail, communication, and professional writing skills.
Responsibilities
  • Provide virtual care, primarily through Cognitive Behavioral Therapy, to eligible patients diagnosed with cancer.
  • Provide individual patient visits addressing mental health needs and concerns.
  • Screen for unmet care needs, including clinical or medication issues, behavioral concerns, safety risks, and psychosocial well-being.
  • Use assessment tools and templates to support evaluations and improve them as needed.
  • Establish trusting relationships with patients and caregivers using active listening and empathy.
  • Maintain close contact with caregivers to monitor side effects and changes in patient function or behavior.
  • Support providers in developing medication management strategies.
  • Consult with licensed clinical team members, including patients' primary care providers, and triage complex or medical issues as needed.
  • Participate in weekly patient rounds with the interdisciplinary care team.
  • Document patient and caregiver interactions in the electronic health record system.
  • Integrate collaborative care programming into the broader care model.
  • Create care plans addressing service gaps, including palliative care, advance care, and end-of-life planning.
