Requirements
  • Active, unrestricted New Jersey registered nurse and Nurse Practitioner licenses, with the ability to obtain additional state licensure.
  • At least 2 years of nursing experience providing care to complex, seriously ill patients.
  • Adult Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Family Nurse Practitioner certification.
  • Graduation from an accredited Nurse Practitioner program.
  • Drug Enforcement Administration licensure, or the ability to obtain it.
  • Advanced Certified Hospice and Palliative Nurse certification, or the ability to obtain it.
  • Willingness and ability to work Monday through Friday from 8:30 a.m. to 5 p.m. Eastern Time, both in the field and remotely, with rotating evening and weekend on-call coverage.
Responsibilities
  • Independently perform comprehensive and symptom management visits to address each patient's individualized needs, along with plan-of-care oversight as defined by state-specific law, focusing on reducing unnecessary hospitalizations and procedures and improving quality of life.
  • Order and interpret diagnostic and therapeutic tests relative to patients' age-specific needs when needed for patients with less than one year of life expectancy.
  • Prescribe appropriate pharmacologic and non-pharmacologic treatment modalities.
  • Collaborate with patients, families, caregivers, and clinical staff to coordinate care across settings, simplify the patient experience, and improve outcomes.
  • Conduct goals-of-care discussions and prognostication, and complete advance care planning documentation, including Physician Orders for Life-Sustaining Treatment forms, with patients and families.
  • Complete transition-of-care visits following discharge from emergency rooms, hospitals, or skilled nursing facilities, reducing readmission risk through warm handoffs and frequent communication.
  • Facilitate transitions to hospice and other healthcare settings as appropriate.
  • Collaborate daily with the interdisciplinary team to review urgent clinical issues, transitions of care, and high-risk patients.
  • Serve as a clinical resource for patients, families, caregivers, and the interdisciplinary team by providing education, anticipatory guidance, and connections to appropriate community and support resources.
  • Apply familiarity with Medicare coding requirements, including fee-for-service Current Procedural Terminology, International Classification of Diseases, Tenth Revision, Hierarchical Condition Category, advance care planning, palliative care, and chronic care management documentation standards.
  • Participate in all required meetings and trainings.
  • Participate in the after-hours on-call rotation.
