- An active Ohio registered nurse or licensed practical nurse license is required.
- A minimum of 3 years of clinical nursing experience is required.
- At least 1 year of home health experience is required.
- Strong communication, critical thinking, and organizational skills are required.
- A valid driver's license and reliable transportation are required.
- Provide clinical leadership and support to nurses, aides, therapists, and social workers.
- Coordinate patient care, clinician assignments, and referrals.
- Oversee care plans to ensure quality outcomes and regulatory compliance.
- Review clinical documentation and support timely billing and reimbursement processes.
- Monitor high-risk patients and collaborate with clinicians to reduce hospitalizations.
- Conduct chart audits and participate in quality improvement initiatives.
- Serve as a clinical resource and mentor for field staff.
- Support staff onboarding, education, and competency development.
- Step into leadership responsibilities in the absence of the Clinical Director or Manager.
- Previous supervisory or leadership experience is preferred.
