Requirements
  • High school diploma or equivalent
  • LPN license or previous experience as a MA
  • Experience in a primary care or comparable care setting
  • Experience documenting clinical data in an electronic health record system
  • Experience working with an ACO and completing activities related to value based care payer contracts
  • Working knowledge of healthcare insurance
  • Strong written and verbal communication skills
  • Knowledge of medical terminology, procedures, and diagnoses
  • Knowledge of safety, OSHA, and infection control protocol
  • Detail oriented and ability to prioritize tasks

Responsibilities
  • Assess patient needs and communicate appropriately to the provider
  • Keep flow of patient care moving in an efficient manner to accommodate the best use of the provider's time with the patient
  • Accurately enter information in patients' charts
  • Prepare and administer injectable medications
  • Assist providers with treatment and care of patients
  • Answer phones professionally and process phone requests/messages in a timely and efficient manner
  • Communicate with providers, patients and payers to process referrals and obtain prior authorizations in an effective manner
  • Assist with performing value based care activities such as reviewing patients' charts to ensure all preventative health screenings and tests/services necessary to close patient care gaps have been completed and contacting patients after ED visits and hospital discharge
  • Other duties as required by management to support clinic operations
