Responsibilities
  • Support and promote the mission and values of Covenant Health Ministry.
  • Work with the inpatient team to identify patients needing referral to Strong Futures services.
  • Attend treatment team meetings.
  • Help ensure seamless transitions of care from the inpatient team to Strong Futures by arranging and scheduling appointments.
  • Support and educate patients and families regarding transitional supports and community resources, including medical, dental, food-SNAP, childcare, and other identified needs.
  • Collaborate and communicate with patients' community case managers while they are in the Strong Futures Program.
  • Assist patients and families with Medicaid requirements and coverage renewals.
  • Contact community resources routinely to establish availability and referral capacity.
  • Communicate professionally with referral sources and return phone calls promptly.
  • Collect patients' medical and behavioral histories for presentation to referral sources while following HIPAA guidelines and system policies.
  • Attend patient appointments with medical and behavioral clinicians as needed.
  • Work with the unit teacher, social worker, and school systems to help patients re-enter school and attend IEP or 504 plan meetings as needed.
  • Document individualized plans for patients when necessary, including goals and outcomes.
  • Review treatment plans with patients and families.
  • Document progress toward plan objectives.
  • Document contact with providers, patients, and families.
  • Manage a dynamic caseload and ensure that patients actively participate in care.
  • Communicate regularly with the clinical supervisor regarding individual patient progress and treatment issues.
  • Attend and participate in individual and team meetings with the clinical supervisor.
  • Inform the clinical supervisor of conflicts of interest, client crises, and other situations requiring further review or assessment.
  • Attend and actively engage in program meetings.
  • Follow up with families after missed appointments when needed.
  • Contribute to a welcoming environment and assist students in their educational experience.
  • Participate in community outreach activities.
  • Partner with unit staff and Strong Futures care providers to conduct school and community marketing visits.
  • Perform other duties consistent with the role.
Desired Qualifications
  • Crisis Prevention Institute training is preferred or must be obtained before completion of orientation.
