- An active registered nurse license in the state in which the nurse practices, or New Grad eligibility to take the NCLEX.
- Conduct Start of Care comprehensive assessments and complete OASIS documentation in a timely manner.
- Draft care plans, reconcile patient medications, and ensure orders for care are accurate and complete.
- Prepare accurate, orderly, and timely documentation.
- Coordinate care with the patient and provide proper reports to the Case Manager.
- Provide health care instructions to the patient and family as appropriate according to the assessment and plan.
