Requirements
  • Graduate of an accredited diploma, associate, or baccalaureate school of nursing.
  • Hold a current state license as a Registered Nurse.
  • Have a minimum of 2 years of experience as a registered nurse.
  • Have reliable transportation, a valid driver’s license, and valid automobile insurance coverage.
  • Be able to travel moderately within the service area.
  • Demonstrate excellent observation, verbal, and written communication skills.
  • Understand business management principles and healthcare operations.
  • Know governmental regulations, Medicare and Medicaid requirements, and CHAP standards.
  • Possess advanced nursing skills consistent with accepted clinical standards.
  • Be able to assess and respond to diverse needs across varied care settings.
  • Have strong organizational skills and the ability to manage multiple priorities and deadlines.
  • Demonstrate empathy when managing emotional situations while respecting individual lifestyles.
  • Have leadership abilities, including coaching, personnel management, and staff development.
  • Demonstrate critical thinking and problem-solving skills.
  • Understand hospice philosophy and palliative care principles.
Responsibilities
  • Lead hospice care delivery with clinical excellence and compassion.
  • Oversee a multidisciplinary team providing end-of-life care, including nurses, social workers, spiritual care, and other supportive services.
  • Ensure each patient receives coordinated, high-quality support that honors dignity, comfort, and individual needs.
  • Guide and develop the interdisciplinary team.
  • Ensure regulatory compliance, quality standards, and operational excellence are consistently met.
  • Partner with field staff, physicians, and organizational leadership to shape care delivery.
  • Support staff development and drive continuous improvement across hospice services.
  • Provide on-call administrative phone support for after-hours field staff.
Desired Qualifications
  • Bachelor of Science in Nursing (BSN).
  • Hospice or home health experience.
  • Management or supervisory experience.

Optimal Care provides Medicare-certified home health services for seniors, guided by a nurse practitioner-led care model. A nurse practitioner visits patients at home and develops individualized care plans that cover behavioral health, dementia, geriatric, diabetic, wound, and orthopedic care. This in-home, NP-driven approach offers a broad suite of geriatric-focused programs rather than only post-surgical or rehabilitation services. Its goal is to keep seniors safe and healthy at home by coordinating comprehensive, specialty care in their living environments.
